This office has no records of births occurring prior to the above date except delayed records of births for persons born before 1911 which have been established by affidavits and documentary evidence. Information needed to obtain a certificate: - Full name at birth of the person for whom the certificate is requested
- The month, day and year of birth
- The county of birth
- The mother's maiden name
- The father's name
